CODE OF CONDUCT FOR PATIENTS AND CLIENTS
Legion Rehabilitation and Adaptive Fitness is committed to providing high quality healthcare and exceptional patient/client experiences. Everyone should expect a safe, caring, and inclusive environment with me.
The Patient and Client Code of Conduct helps all of us to meet this goal.
Many healthcare systems and clinics across the country have similar codes of conduct in order to guarantee the best patient/client care experience.
You can find a copy of this code of conduct in the consent forms, which require your attestation prior to becoming a patient or client of Legion Rehabilitation and Adaptive Fitness.
Words or actions that are disrespectful, discriminatory, hostile, or harassing are NOT WELCOME.
Examples of these include:
Offensive comments about others’ race, accent, religion, gender, sexual orientation, or other personal traits
Physical or verbal threats and assaults
Sexual or vulgar words or actions
Disrupting another patient or client’s care or experience
Harassment of Legion Rehabilitation and Adaptive Fitness personnel online through any social media platform such as Facebook or Instagram, Google or Yelp reviews, over text or call or email, and in-person will not be tolerated in any form or kind.
Financial Responsibility:
At Legion RAF, it is understood that patients and clients can experience delays in securing payment for services rendered due to a number of factors, including but notwithstanding:
Changes to payment method
Payment and income schedules
Relocation of address
Changes to employment status
If you have experienced any of the above or an unlisted circumstance which has resulted in your inability to pay for services rendered, please contact me directly at audrey@legionraf.com and I will work with you to resolve this issue.
ANY PATIENT OR CLIENT FOUND TO BE DODGING PAYMENT FOR SERVICES RENDERED WILL BE TERMINATED AS A PATIENT OR CLIENT IMMEDIATELY AND WILL NOT BE ELIGIBLE FOR FUTURE SERVICES FROM LEGION REHABILITATION AND ADAPTIVE FITNESS.
If I believe you have violated the Code in part or whole, you will be notified and given the chance to explain your point of view in a meeting with me and/or other affected parties.
I will always carefully consider your response before I make any decisions about future care at Legion RAF.
Some violations of this Code may lead to patients and clients being asked to make other plans for their care and future care at Legion RAF. This may require review, though I expect this to be rare.
I reserve the right to terminate any sessions, plans of care, training plans, and other services, without providing a refund for services already purchased by the patient, should the patient in question be found to have violated any part of this Code of Conduct.
